Here’s what you need to bring:
- A Driver’s License for identification
- Two current proofs of residency (like utility bills) with your 911 physical address
- Updated shot records
For new enrollees, please also bring:
- Original birth certificate
- Shot record
- Withdrawal form from the previous school
- Last report card showing pass/fail
